Saint Joseph Catholic Church Anniversary Request Form

Please fill out the form providing all required information, and your anniversary will be celebrated in the Church bulletin and e-news. Anniversaries are noted in the bulletin and e-news monthly. Requests must be submitted no later than the 15th day of the previous month of the couple's Anniversary to be included in Church publications the month of their Anniversary.
