If you need to obtain a copy of a sacramental record, complete the Sacramental Record Request form in its entirety and submit it below. Please allow at least one week for the certificate to be prepared and signed by the pastor. Sacramental records requests will only be taken using the online form below. If you have questions regarding the form or need additional information, please contact the Parish office at 770-422-5633.

Please note, parents are responsible for providing copies of the baptismal certificate when enrolling their child in religious education programs and preparing them for First Eucharist or Confirmation.
